SVR4C Confidentiality Policy
Consumer Education and Referral

Respect of Confidentiality:

Information relating to customers, employees, and contracts of SVR4C must not be released to other persons, agencies, or programs unless written authorization is provided by the appropriate person. Employees must be responsible for what they say and do regarding the confidential information they may have access to as they do their job.

Personal information regarding customers and employees of SVR4C should be divulged to other employees only to the extent of what they need to know to support the person or program.

Any breech of confidential information can result in immediate termination.

The following activities are prohibited:

  1. Copying of confidential client (parent, provider, employer, contractor) information for outside use or the release, removal, or discussion of such information with anyone not immediately concerned with service delivery;
  2. Providing personal contact information of coworker without their permission.


Shredding of Confidential Documents at SVR4C:

SVR4C shreds all paperwork and documentation that includes customer information upon understanding that these items are no longer needed at the agency. This paperwork is deposited in an onsite, locked shredding container, and is shredded quarterly by a company called, "Shred-It: A Confidential Paper Shredding & Recycling System."
